Emergency Medical Transportation: If you need emergency medical transportation, please call “911”.   

Non-Emergency Medical Transportation: Your primary care physician (PCP) or specialist will need to complete a Provider Certification Statement form prior to receiving transportation. Please call American Logistics at 1 (844) 292-2688. American Logistics accepts requests 24 hours a day, 7 days a week. We recommend calling at least 3 business days in advance of your appointment. Or call as soon as you can when you have an urgent appointment. Please have your member ID card ready when you call.
